Immunization Requirements

  • Parents of current PISD students may log into Skyward Family Access to view their student’s immunization record. To request and official immunization record for an actively enrolled student, contact the school where your child is enrolled.

    To request immunization records for a previously enrolled Pasadena ISD student, please complete this FERPA Release of Health Records Form and email it along with a copy of your photo ID to the Health Services department at DCastillo@pasadenaisd.org. For further questions regarding student health records, please contact Health Services at 713-740-0149.
